Application for Continuing Education Credit

A total of 5.0 clock hours of continuing education have been approved for the completion of
these ten modules. To obtain the maximum hours you are required to complete this
application, answer all the questions, obtain a score of 80% or higher on each module, and
complete the evaluation form. Only original, completed packets will be scored. CRCC will not
accept photocopies for continuing education credit. A fee of $65.00 will be assessed for the
course. This fee must be paid at the time the materials are submitted to CRCC for evaluation
and addition to your certification file. Please make your check payable to CRCC.
