Disclosure Forms

CRCC's Ethics Committee completed a survey on the professional disclosure practices of CRCs. While most support the need for professional disclosure, there is great disparity in the content and manner in which information is conveyed. CRCC recommends the use of written professional disclosure and release statements and offers the following sample forms.

The forms below require review for applicability and modifications must be made as appropriate. Revisions for releases may be required based on various laws or based on HIPAA requirements for transfer of medical records.
